Position: Director of Resident Assessment (RNAC)

We are currently seeking a highly skilled and experienced Director of Resident Assessment (RNAC) to join our team at Chestnut Hill Lodge Health And Rehab Ctr. In this role, you will be responsible for coordinating and overseeing the resident assessment process to ensure accurate and thorough evaluations. You will work closely with our interdisciplinary team to identify the needs of our residents and develop individualized care plans.


Responsibilities:

  • Coordinate and conduct timely and comprehensive resident assessments
  • Collect and analyze relevant data to determine the level of care required
  • Identify residents' physical, emotional, and social needs
  • Collaborate with the interdisciplinary team to develop individualized care plans
  • Ensure accurate and up-to-date documentation of assessments and care plans
  • Monitor and evaluate the effectiveness of care plans and make adjustments as necessary
  • Provide guidance and support to staff in implementing resident care plans
  • Stay current with industry regulations and best practices in resident assessment
  • Participate in quality improvement initiatives

Requirements:

  • Valid Registered Nurse (RN) license
  • Proven experience in resident assessment in a healthcare setting
  • Strong knowledge of federal and state regulations related to resident assessment
  • Excellent analytical and critical thinking skills
  • Exceptional attention to detail
  • Effective communication and interpersonal skills
  • Ability to work collaboratively in a team environment
  • Proficient computer skills
