Job Description
Kline Galland's Community-Based Services provide one-to-one health care for seniors living throughout the Puget Sound region. We customize our services and provide them when and where our clients need them most, often in the comfort of their homes. Join our team as a Hospice RN Case Manager and be part of our Continuum of Care that is never-ending.
Your key responsibilities will be:
- Performing comprehensive assessments and developing care plans in collaboration with patients and their families
- Providing case management, including education, coordinating resources, delegating to other team members, pain and symptom management, and skilled medical interventions
- Working closely with patients and their caregiving team to maintain regulatory compliance
- Honoring patients and their families with compassion and care at this pivotal time
If you desire to join our team, you will need:
- RN degree from an accredited school
- Current RN license
- Valid driver's license, auto insurance, and a dependable vehicle
- Prior experience in hospice, oncology, or home health (experience not required but preferred)
