MDS Coordinator (Registered Nurse/RN) $10,000 SOB | Life Care Center of Melbourne

The Life Care Center of Melbourne is currently seeking a dedicated MDS Coordinator (Registered Nurse/RN) to join our team. This position offers a $10,000 sign-on bonus.

Key Responsibilities:

• Coordinates and assists with completion and submission of accurate and timely interdisciplinary MDS Assessments, CAAs, and Care Plans for all residents.
• Assists in the development and implementation of resident care plans and assessment protocols.
• Conducts or coordinates training for team members regarding MDS documentation and care planning, ensuring adherence to company policies and procedures.
• Participates in quality assurance activities to ensure appropriate documentation and assessment for resident care.
• Interacts with medical staff, nursing staff, and other departments and agencies in order to provide comprehensive care to residents.
• Adheres to and supports Life Care Center of Melbourne’s standards of care, policies, and procedures.

Qualifications:

• Must be a Registered Nurse (RN) with required licensure in the state of Florida.
• Previous MDS experience and working knowledge of MDS process and care planning is preferred.
• Strong analytical and critical thinking skills, as well as attention to detail and accuracy in documentation.
• Excellent communication skills and the ability to work as part of a multidisciplinary team.
