Job Description:
Renaissance Rehabilitation And Healthcare Center is seeking a skilled and compassionate LVN/LPN to join our dedicated healthcare team. As an LVN/LPN, you will play a crucial role in providing high-quality nursing care to our residents.
Responsibilities:
1. Nursing Care: Provide direct patient care under the supervision of registered nurses. Administer medications, assist with treatments, and document care.
2. Assessment and Monitoring: Conduct routine assessments, monitor patient's vital signs, and identify any changes in condition. Report findings to the registered nurse.
3. Collaborate with Healthcare Team: Communicate effectively with physicians, registered nurses, and other healthcare professionals to ensure comprehensive and coordinated care for residents.
4. Document Care: Maintain accurate and detailed documentation of patient care activities, including observations, interventions, and outcomes.
5. Resident Support: Provide emotional support and engage in therapeutic communication with residents and their families.
6. Compliance: Adhere to all regulatory and company policies, procedures, and protocols to ensure patient safety and quality care.
Requirements:
1. Education: Graduation from an accredited program of licensed vocational nursing.
2. Licensure: Current, active LVN/LPN license in the state.
3. Experience: Prior experience in a skilled nursing facility or long-term care setting is preferred.
4. Communication Skills: Excellent oral and written communication skills.
5. Team Player: Ability to work collaboratively with a diverse team of healthcare professionals.
6. Compassion: Genuine empathy and compassion for residents and their families.
7. Attention to Detail: Strong attention to detail and accuracy in documentation and medication administration.
8. Adaptability: Ability to adapt to changing patient needs and priorities.
